Youth Protection Standards for Volunteers

 Please return the Volunteer Enrollment Forms as soon as possible.
The High Responsibility Volunteer and Advisory Volunteer Form are due to the State Office immediately.  Please fill in all information on both sides.  The 4-H Leaders are paying for the $10 Cost Recovery Fee.

Liability Insurance is included in the $10 fee for the High Responsibility Volunteers. 
All other volunteers will be covered with the May renewal for $1.72
Medical coverage will be the same as 4-H Members - $1.50 or $2.50

 If you have problems with the fillable forms below, try using Internet Explorer as your browser.  Then you will be able to save the form to your computer.  We find if we use Foxfire or Google Chrome, it does not allow you to save a copy.

4-H Record Book Annual Report (Record Book) 
Due October 1

Accumulative Record Pages
Accum Pages Fillable format
for public presentation, fashion revue, special foods, judging, showmanship, other awards

Permanent Record (fillable)
tally points for achievement awards and pins; keep for all years in 4-H
Great snapshot of your 4-H career.  Must be turned in with your record book

(Livestock/Small Animal Financial Form is no longer required)
